Required Student Health Insurance

Wake Forest University requires health insurance as a condition of enrollment for full-time, degree-seeking domestic graduate students, and all international students with F or J visas. Students who demonstrate comparable coverage may waive the insurance provided by WFU. Information on the waiver process can be found on the Enrollment, Verifications & Waivers page.

All students are REQUIRED to either (1) Complete the enrollment form for the WFU health insurance plan, or (2) Provide proof of coverage and waive the WFU student health insurance by the 2014 deadline. Check the criteria to waive coverage. Go to the Student Insurance Information page at http://sip.studentlife.wfu.edu/ to complete the enrollment or waiver process. You will need your student I.D. number, which will be included in the letter from the I.T. Department. Optional coverage for the summer semester (May-July 2014) is also available.

INTERNATIONAL STUDENTS: International students with F or J visas will not have the option to waive from the Student Blue plan unless they are covered by a plan reviewed and approved by the University based on the minimum criteria. Check the criteria to waive coverage. Additionally, the plan must be provided by a company licensed to do business in the U.S. with (a) a U.S. claim payment office, (b) a U.S. telephone number, and (c) plan literature available in English.

Required Immunizations

Health and Immunization Forms are REQUIRED for all incoming students. Complete and mail the University Health & Immunization form by July 1! If your immunization record is already on file with the University Health Services Office, you need only submit pages 1-3 of the form. If you are admitted after July 1, please complete and mail within two weeks of submitting your deposit. IMPORTANT: If Student Health Services does not have your completed health and immunization form, you will not be permitted to attend classes!
